Zusammenfassung
Hintergrund Das TINU-Syndrom (Tubulo-interstitielle Nephritis und Uveitis) ist eine seltene immunmediierte
Erkrankung, welche vorwiegend bei Kindern und jungen Frauen beobachtet wird. Die Uveitis
kann vor, nach oder gleichzeitig mit der Nephritis auftreten. Therapeutisch werden
Kortikosteroide eingesetzt.
Patientin Bei einer 73-jährigen Patientin wurde bei tubulo-interstitieller Nephritis und rezidivierenden
vorderen Uveitiden die Diagnose eines TINU-Syndroms gestellt. In der Folge trat ein
bilaterales zystoides Makulaödem mit entsprechendem Visusabfall auf. Grunderkrankung
und Komplikationen sprachen gut auf systemische Steroide an.
Schlussfolgerung Die internistische Uveitisabklärung sollte in jedem Lebensalter assoziierte Nierenerkrankungen
ausschließen. Genaue Anamnese, klinische Untersuchung und entsprechende Labortests
(Nierenparameter, Urinstatus) können zur Diagnose führen, häufig ist jedoch eine Nierenbiopsie
zur exakten Diagnosestellung unumgänglich. Langfristige ophthalmologische Kontrollen
sind erforderlich, um Komplikationen erfolgreich zu therapieren.
Background TINU - Syndrome (Acute tubulointerstitial nephritis and uveitis) is a rare immune-mediated
disease, primarily occurring in children and young women. Uveitis may precede, follow
or occur concomitantly with nephritis. Corticosteroids are the mainstay of therapy.
Patient A 73-year old female patient with tubulointerstitial nephritis and recurrences of
anterior uveitis was diagnosed having TINU syndrome. During follow-up she developed
bilateral cystoid macular edema with accompanying visual loss. Nephritis and uveitis
responded well to oral corticosteroids.
Conclusions Uveitis screening should exclude renal disease independent of the age of the patient.
Diagnosis can often be inferred from anamnesis, physical examination, or laboratory
tests. Renal biopsy as the most definite method of diagnosis may be necessary. Ophthalmologic
follow-up on a regular basis is mandatory for a successful management of long-term
complications.
